Teamsters Rx - Pharmacy Benefit Manager and Mail-Order Pharmacy

Pharmacy Benefit Manager

Effective May 1, 2005 your prescription benefit will be managed by Teamsters Rx. You should have received new prescription ID cards in April. Please discard your Caremark cards and present your new Teamsters Rx prescription card to your pharmacist so that your coverage information may be updated. Your new Teamsters Rx card may be used at most major retail pharmacy chains across the country except Wal-Mart and Sam’s Club. Your present co-pays and managed drug limitations remain the same.

Mail Order Pharmacy

Effective October 3, 2005 all mail-order pharmacy orders will be filled at the new Teamsters Rx Pharmacy in Manchester, NH. This pharmacy will be mail-order only with no walk-in services available. Your pharmacy benefits and co-pays will remain the same. The drug/dosing limitations also remain in effect. In your recent mailing, you would have received the new mail-order pharmacy information along with a new mail-order envelope. . Please provide the new Teamsters Rx Pharmacy with your debit/credit card information. . By having a debit/credit card on file in the pharmacy, you may call in refills 24 hours a day instead of mailing in your envelope. A patient profile form was also included. The information contained in the profile will help the pharmacy team serve you better as they will be aware of your medical condition and allergies. The new toll-free Teamsters Rx Pharmacy number is 866-888-0104. This number will give you access to a pharmacist if you have questions about your prescription, confirm when your prescription was filled and mailed plus your doctor may call/fax in a new prescription if your debit/credit card is on file.
