How do I find a dentist covered under my plan? |

NNEBT does not have a participating provider network for dental benefits. You may see any licensed dentist in your area. Please consult the Dental Fee Schedule for the maximum fees NNEBT will pay.

Eligibility |
My dependent child is over the age of 19 and still a full-time student. What do I need to do to maintain his/her coverage? |

If your dependent child is between the ages of 19-23 and a full-time student, he/she may remain on your plan. The only way this can be done is by providing a letter of student verification from the institution they are attending. All members with dependent children between 19-23 will receive a letter from Northern New England Benefit Trust outlining the rules and what process must be followed. See the Eligibility Section of your Summary Plan Description. |

How do I add or remove dependents from my insurance? |

You can add or remove qualified dependents from your plan any time there is a “qualifying event” (birth, death, marriage, divorce, loss of other coverage) or during your employer’s open enrollment period. You should contact NNEBT’s enrollment department and we will send you the appropriate forms to add or remove your dependent. You should also notify your employer of the change.

Medical |
I received an Explanation of Benefits that I believe is incorrect. What should I do? |

You may contact your healthcare insurance company by calling the Member Services Number on the back of your medical card. A customer representative will review the EOB (Explanation of Benefits) and advise you as to how and why the charges were processed. In some cases, the medical provider (doctor’s office) may have to be contacted to review the claim that was submitted to the insurance company.

Pay-In Notices |
When will I become eligible again if I do not pay my Pay-In? |

If you choose not to pay in, your coverage will terminate on the date indicated on your pay-in notice. The only way you can be reinstated once your coverage terminates (and the due date for the pay-in option has passed) is by meeting the reinstatement requirements (see your Plan’s eligibility rules for reinstatement requirements ). Please note: there is no pay-in option for reinstating your coverage once you allow your coverage to terminate so it is possible that you could remain ineligible for an indefinite period of time if you continually work fewer than the minimum required hours to reinstate. |

Who do I notify of an address change? |

You should call NNEBT to report an address change. NNEBT will then update the various other vendors (Cigna, Blue Cross, Davis Vision, etc.). |
